No, man this is fucking simple. Have her call your phone and do not find it... Then after the gym, get your phone and text her:
"Hey, is this _____ from the gym? I found my phone after all on my way to work. I left it in my car of all places! Thanks a lot!
Do not hit on her in the first text message. Then she'll reply to you. If she doesn't, then that's it, she didn't like your performance. If she does reply, 'you're welcome' etc.
Tell her that you owe her a favor for helping you out and this is where you continue your conversation from when you were at the gym. This is also a perfect opportunity to treat her to a dinner or something else.
